Hawaii Permanente Medical Group, Inc. is seeking an experienced Internal Medicine or Family Medicine physician to join its Continuing Care Department at Kaiser Permanente Moanalua Medical Center, a tertiary care center located 5 miles from downtown Honolulu.

The role involves caring for patients in post-acute, hospice and palliative care with a multi-disciplinary team and a flexible schedule designed to support work-life balance in a tropical setting.

Formación

• Board-certified or clinically experienced in Internal Medicine or Family Medicine with hospice/palliative focus.
• Minimum 5 years of experience preferred.

Responsabilidades

• Care for patients in post-acute, hospice and palliative settings within a continuing care department.
• Collaborate with a multi-disciplinary care team to coordinate patient plans.
• Provide compassionate, patient-centered care across settings.
