Job summary

One Home Health LLC serves Porter County and surrounding Northwest Indiana communities. We are seeking an Experienced Home Health Physical Therapist to evaluate patients in their homes and provide personalized care, improving mobility and independence.

This part-time/PRN role offers flexible schedules, competitive pay of $60–$80 per hour, and opportunities for professional growth within a supportive interdisciplinary team. Documentation and team collaboration are essential.

Qualifications

• Active Illinois Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Excellent communication, organization, and time-management skills.
• Ability to work independently in a home health setting.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients’ homes.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Provide skilled physical therapy interventions according to physician orders and established care plans.
• Assess patients’ home environments and recommend safety modifications and assistive devices as appropriate.
• Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
• Monitor patient progress and adjust treatment plans based on clinical findings.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in discharge planning and patient outcome evaluations.
