Responsibilities:
  1. Gather clinical information via phone, using clinical guidelines to assess needs and direct appropriate care or referrals.
  2. Provide triage and direct client care according to state nurse practice acts.
  3. Educate and counsel clients and families, ensuring effective communication and reassurance.
  4. Coordinate with healthcare providers and community resources to optimize care and prevent rehospitalizations.
  5. Document clinical notes and communicate client status to providers.
  6. Participate in training, team collaboration, and safety protocols.
  7. Maintain confidentiality and adhere to legal and organizational standards.
Qualifications:
  • Graduate of an accredited nursing program with a current Minnesota license.
  • 1-2 years of recent acute care, triage, or home health experience, with knowledge of triage in clinics or SNFs.
  • Strong communication, technology proficiency, and problem-solving skills.
  • Ability to work independently and as part of a team, with a focus on customer service and client-centered care.
  • Verification of COVID vaccination or approved exemption.
