Southeast Primary Care Partners is seeking a Population Health Transformation Specialist to lead the transition to value-based care across multiple clinics, serving as the primary partner for providers, managers, and staff at each location on all matters related to population health.

You will drive development and implementation of pathways, provide ongoing training on quality measures, RAF reporting, and strategies for closing care gaps, and perform frequent site visits to support providers in

Qualifications

• Experience with value-based payor programs and HEDIS/STARS
• Experience training clinical staff on value-based programs
• Proficiency with EHRs and documentation workflows
• Ability to audit charts for gap-closure
• High school diploma; associates preferred

Responsibilities

• Lead transition to value-based care across multiple clinics
• Serve as primary partner for providers, managers, and staff on population health matters
• Develop and implement care pathways supporting value-based care
• Provide ongoing training on quality measures, RAF reporting, and care gaps
• Travel to assigned practices to support providers and staff
• Review performance data and create action plans for improvement
• Collaborate with practice directors and clinical teams to drive initiatives
