Job summary

Southeast Primary Care Partners seeks a Population Health Transformation Specialist to lead the shift to value-based care across multiple clinics, partnering with providers, managers and staff on all aspects of population health.

You will conduct site visits, develop and implement care pathways, track performance and provide ongoing training on quality measures, RAF reporting and care-gap strategies, while staying current with HEDIS and STARs guidelines.

Qualifications

• Experience with value-based payor programs and HEDIS coding.
• Experience with HEDIS and STARS ratings.
• Allscripts/Veradigm experience preferred.
• Experience training clinical staff on value-based programs.
• Auditing documents for gap-closure.
• High-school graduate; associates preferred.

Responsibilities

• Lead transition to outcomes-based care across multiple clinics.
• Partner with providers, managers and staff on population health.
• Develop and implement pathways to support value-based care.
• Train teams on quality measures, RAF reporting and care gaps.
• Review and interpret performance data for practices.
• Meet project goals and timelines.
