Job summary
Trillium Health Resources is seeking a Quality Management Specialist III - Population Health to develop policies, oversee program quality, and provide expert guidance on special populations. The role includes data-driven analyses, staff education, and coordinating care management services across North Carolina.
The ideal candidate will hold NC-licensed credentials (Nurse or equivalent) and have at least 1 year of experience with complex health conditions, with willingness to travel within the
Qualifications
- Bachelor's degree in Nursing or Master’s in Health/Human Services with 1 year of relevant care experience.
- Fully licensed by NC as LCSW/LCMHC/LPA/LMFT/LCAS or RN.
- Valid NC driver's license required.
- Must reside in North Carolina and able to travel within catchment.
Responsibilities
- Develop, review and evaluate operating policies, practices and procedures with department-wide impact.
- Analyze operations using databases and dashboards; propose changes to meet program goals.
- Collaborate with internal teams to identify workflow improvements for special populations.
- Serve as subject matter expert on identified populations; provide staff education and training.
- Oversee resources like secure transportation for special populations.
