Job summary

Tanner Clinic seeks a Population Health Coordinator to support value-based care and quality improvement by identifying and resolving gaps in patient care. You will review EHR data, verify preventive services, and help ensure accurate quality reporting.

Collaborating with physicians and staff, you will coordinate follow-up on identified care gaps and assist with population health workflows to improve preventive and chronic care outcomes.

Qualifications

• Knowledge of medical terminology preferred.
• Understanding of preventive care and quality measures preferred.
• Familiarity with HIPAA and patient confidentiality requirements.
• Experience with population health management and quality reporting preferred.

Responsibilities

• Support value-based care goals and performance on quality measures.
• Review population health reports to identify care gaps.
• Coordinate follow-up on identified gaps per workflows.
• Maintain accurate documentation and assist with quality reporting.
• Collaborate with physicians and staff to address missing information.
