Job summary

Qualifications

• High school degree or equivalent, with strong communication and organizational skills.
• Two years of customer interactions via phone and marketing/communications preferred.
• Active user of electronic medical records software strongly preferred.
• An associate's degree or higher may be substituted for experience.

Responsibilities

• Provide outreach and care management support to patients and educate clients on available resources.
• Coordinate care between healthcare providers and complex care teams.
• Document activities, plans, and follow‑ups in medical records and systems.
• Attend outreach education and recruitment sessions.
• Promote program with coalitions and coordinate community resources.
