• Ability to solve problems independently.
• Knowledge of MyChart and patient access workflows.
• Recognize red-flag symptoms and escalate when needed.
• Background check prior to employment.

Responsibilities

• Support quality improvement, outreach, clinical operations, and population health initiatives.
• Conduct patient outreach to identify and resolve barriers to care.
• Communicate with care teams across the organization.
• Lead or support clinical operations workflow development and improvement.
• Assist Quality Improvement/Population Health Managers with meetings, data collection, analysis, and workflow activities.
• Build and maintain relationships with key stakeholders.
• Collaborate to improve patient care and outcomes.
• Implement and monitor workflow and screening fidelity.
• Oversee volunteers for outreach activities.
• Use Reporting Workbench and other reports to assess outreach needs and close care gaps.
• Perform and document patient outreach per NeighborHealth protocols.
• Provide EMR and technology training; be the resident expert on assigned applications/tools.
• Handle email correspondence and non-call-related tasks.
• Navigate EPIC to manage calls, appointments, messages, and refill requests.
• Participate in departmental and organizational meetings and activities.
• Adhere to policies and procedures.
