Your Health Organization in Marietta, GA seeks a dedicated Population Health Nurse (RN) to drive population health initiatives across regions and directly manage high-risk patient populations. The role blends data-informed decision-making, clinical case management, and leadership to improve care quality and reduce unnecessary utilization.

This position will be working daily in our Marietta office and is a full-time, salary-based 8-hour shift plan (8:00 AM–5:00 PM, Monday–Friday).

Qualifications

• Licensed RN with current license in good standing; BSN preferred.
• Experience in population health, care management, or home-based/community settings preferred.
• Proficient with EMR systems, telemedicine, remote monitoring, and data-driven care planning.

Responsibilities

• Develop and implement strategies to identify and manage at-risk patient populations using clinical judgment and data analytics.
• Collaborate with executive leadership and analytics teams to improve outcomes tied to value-based contracts.
• Monitor trends in chronic disease burden, hospitalizations, ED use, readmissions, and patient engagement to inform targeted interventions.
• Serve as the clinical lead for regional population health efforts and drive consistency across teams.
• Lead outreach and coordination for high-risk patients, including those with complex medical or social needs.
• Support transitions of care following hospital discharge or acute events, ensuring timely follow-up and appropriate plan of care.
• Coach and mentor staff on documentation, coordination standards, and value-based care concepts.
