Your Health is seeking a Nurse Practitioner to provide in-home and telehealth primary care across the Marietta area, working with a dedicated care team to deliver comprehensive, compassionate patient management. You will evaluate high-risk patients, coordinate plans, and support families with education and resources.

The position is full-time with a salary-based structure and the potential for a monthly bonus.

Qualifications

• Must hold an active NP license in the employing state.
• Experience in home health, telehealth, or clinic settings preferred.
• Strong communication and collaboration within interdisciplinary teams.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with the interdisciplinary team.
• Communicate with case managers, coordinators, and therapists to ensure continuity of care.
• Document care management activities and ensure regulatory compliance.
• Provide patient education and resources for self-management and prevention.
