Job summary

Saint Alphonsus Health System in Boise, ID seeks a Population Health MA to primarily perform proactive patient outreach and close gaps in care. This in-person role supports 180 clinics via the Saint Alphonsus Health Alliance and emphasizes data-driven quality performance for HEDIS measures.

The ideal candidate has MA certification within 90 days, 3–5 years of MA experience, BLS certification, and proficiency with EPIC/EHR. 8–5, Monday–Friday schedule in West Boise location.

Qualifications

• 3 years of MA clinical experience required; 5 years preferred.
• MA certification within 90 days of eligibility to take a national certification exam from Saint Alphonsus approved agencies.
• High School Diploma or equivalent required.
• BLS for Healthcare Providers certification required at hire.
• EHR experience required.
• Experience with process improvement or Lean philosophy preferred.

Responsibilities

• Primarily contacting patients by phone to close gaps in care and support quality performance goals (HEDIS/STAR).
• Assist with scheduling health screenings and follow-up testing as needed.
• Analyze preventive care screenings and review payor lists for open gaps in care.
• Perform chart reviews to ensure accurate documentation of patient complexity.
• Collaborate with primary care teams to support quality improvement initiatives.
