Qualifications

Responsibilities

• Perform structured telephonic outreach to newly attributed patients within designated timelines.
• Facilitate appointment scheduling for ≥30% of contacted patients.
• Ensure 100% documentation of outreach attempts and patient responses.
• Produce, monitor, and analyze outcome reports related to engagement timelines and visit completion.
• Identify trends, gaps, and opportunities for improvement; escalate as needed.
• Prepare regular performance summaries for leadership and population health teams.
