Gibson Area Hospital & Health Services seeks a Population Health Coordinator to advance data-driven care within its Accountable Care Organization. This role focuses on extracting and managing patient data from EHRs, developing dashboards, and supporting chronic disease coordination and AWV initiatives.

The position emphasizes data accuracy, reporting to leadership, and collaboration with care teams to improve patient outcomes and reduce avoidable utilization across the organization.

Competenze

• Degree in healthcare administration, health information management, data analytics, business administration, or a related field preferred; equivalent relevant education and experience may be considered.
• Experience in healthcare settings, medical records, data management, quality improvement or administrative support preferred.
• Experience working with electronic health records, spreadsheets, databases or reporting tools preferred.
• Demonstrate high attention to detail and a high degree of accuracy

Mansioni

• Medical Data Extraction and Management: extract patient-level data from EHRs and related systems; maintain registries and databases.
• Data Analysis and Reporting: analyze data to identify trends and prepare routine and ad hoc reports.
• Quality Improvement and Program Support: support initiatives to improve outcomes and care coordination.
• Other duties as assigned to support departmental and organizational objectives.

Formazione
