Job summary

Gibson Area Hospital & Health Services in Gibson City, IL seeks a Population Health Coordinator to support the ACO by extracting and analyzing patient data, improving care coordination, and reporting on chronic disease management and preventive services.

You will work under the ACO Director, manage data quality, develop dashboards, and assist with Medicare AWV and other initiatives. This onsite, full-time role focuses on improving outcomes and reducing avoidable hospitalizations.

Qualifications

• Extract patient-level and clinical data from electronic health records and other approved data systems.
• Compile and organize data related to preventive care, chronic disease management, quality measures, utilization, patient outcomes, and other population health priorities.
• Review data for accuracy, completeness, consistency, and potential discrepancies.
• Maintain spreadsheets, registries, tracking tools, and databases used to monitor patient populations and program performance.
• Identify missing, inconsistent, or incomplete data and coordinate appropriate follow-up.
• Handle protected health information in accordance with HIPAA, hospital policies, and applicable privacy and security requirements.
