Job summary

Optum is seeking a data coordination professional to work with patients, providers, and sites to support outreach for chronic illness management. You will help arrange labs, schedule appointments in EMR, and generate population reports to improve care delivery.

Collaboration with clinicians and care teams is essential to optimize patient outcomes. Required is a High School Diploma with strong data handling, Microsoft Office proficiency, and comfort working with EMR systems in a medical office

Qualifications

• High School Diploma/GED.
• Understanding of medical terminology (preferred).
• 2+ years of experience with an EMR system (preferred).
• 2+ years of experience in a medical office setting (preferred).

Responsibilities

• Contacts patients for outreach to stress the clinical importance of lab work and appointments.
• Place lab orders and/or schedule appointments in EMR at the clinician's request.
• Generate reports to identify patients needing labs or overdue for labs.
• Consult with clinicians to determine required population reports or patient lists.
• Coordinate with OPH nurses and care staff to optimize patient care.
• Attend roster reviews and Tier II meetings when appropriate.
• Identify data inconsistencies and opportunities to improve performance.
• Comply with all policies, safety rules, and regulations.
• Take part in meetings, conferences, and educational programs.
• Assist with external test result coordination and EMR data entry.
• Perform other duties as required; maintain regular attendance.
