You will support quality improvement, outreach, clinical operations, and population health initiatives, making a significant impact on underserved individuals and families. Your responsibilities will include developing workflows, engaging stakeholders, overseeing outreach efforts, utilizing reports to assess patient needs, and navigating EMR systems like EPIC and MyChart. Additionally, you will handle patient calls, schedule appointments, document interactions, and ensure compliance with healthcare standards and regulations.
