Responsibilities

• Lead population management by monitoring and sharing data to improve care outcomes.
• Champion proactive care for the population connected with Metro Pediatrics.
• Serve as subject matter expert on quality metrics and participate in quality improvement activities.
• Engage families and patients through outreach and timely follow-ups.
• Maintain provider panels and coordinate with health plans.
• Protect confidentiality of sensitive patient information and work in team settings.
