Qualifications

Responsibilities

• Conduct comprehensive assessments of medical, behavioral health, psychosocial, environmental, financial, and educational needs.
• Develop, implement, and coordinate individualized care plans and interventions.
• Perform disease and chronic condition management for high-risk populations.
• Coordinate care across providers and settings to reduce fragmentation and improve utilization.
