Population Health Care Management Team Lead

The Care Management Team Lead works in coordination with PHMO leadership in an assigned program to ensure that the work of care management staff is accomplished effectively. The role includes supervision of the assigned team, overseeing productivity, performance, and quality, and may involve a small caseload to engage customers within an identified population.

General Description Of The Job Class

The Team Lead may have additional responsibilities beyond the typical job class, providing clinical/functional expertise for specific customer populations, and functioning as an integral part of an interdisciplinary team to achieve seamless transitions of care.

Duties And Responsibilities Of This Level

• Coordinate and facilitate timely implementation of interventions for identified customer populations following established policies and procedures, and recommending changes when gaps are identified.
• Monitor, support, and engage staff and management in systemic opportunities to enhance payer negotiations, improve care management, and address gaps in care.
• Participate in the hiring and firing process of care‑management staff.
• Conduct monthly individual supervision and team meetings with direct reports to provide updates, training, and coaching; provide productivity reports monthly.
• Shadow each direct report at least once per evaluation period unless concerns arise.
• Provide one‑on‑one and group training on workflow, documentation, and patient care.
• Manage staff time and attendance in the API system.
• Complete routine audits in data management and call recording systems for quality, clinical care, and process accuracy; provide timely oversight and feedback.
• Participate in PHMO/DukeWELL meetings and relevant training.
• Develop and maintain positive relationships with internal and external customers.
• Maintain a minor caseload, completing documentation, assessment, medication reconciliation, and care planning within required timeframes.

Education Requirements

Bachelor's degree in a clinical field such as Nursing, Counseling, Social Work, Therapy, Allied Health, or a community health related field.

Required Qualifications at this Level

• 3 years of clinical experience.
• Extensive management experience.
• Care/Case management experience in settings such as Home Health, Community, or General Medicine.
• Strategic Planning experience.

Degree and Certification (one of the following)

• Current or compact RN licensure in the state of North Carolina.
• Current licensure as a Licensed Clinical Social Worker by the NC Social Work Certification and Licensure Board.
• Current licensure as a Licensed Professional Counselor by the state of NC.
• Current licensure as a Licensed Addiction Specialist in North Carolina.
• ACM or CCM certification within 3 years of hire date or by December 31, 2020.
