Job summary

People Incorporated Mental Health Services in Minneapolis seeks a Population Health Care Coordinator to advance whole-person health within outpatient teams. You will coordinate care for clients with co-morbid conditions, engage clients in education and problem-solving, and support population health initiatives using data-driven methods.

Regular hours, full-time with a $1,000 signing bonus for external candidates.

Qualifications

• Master’s degree in a behavioral science or related field is required.
• Bachelor’s degree in a behavioral field with practicum/ internship or direct work experience.
• Alternative: 30 college credits in behavioral sciences plus direct client interaction or 2,000 hours of relevant work.
• Alternative: high school diploma with extensive related work experience and supervision requirements.

Responsibilities

• Integrated Client Care Coordination – coordinate care across behavioral, physical, and mental health domains.
• Client Engagement, Support & Stabilization – engage clients, provide support, link to resources.
• Population Health & Preventative Services – collect data, implement health initiatives, promote wellness.
• Team Collaboration & Communication – work with interdisciplinary teams to resolve client issues.
• Documentation & Professional Practice – document interactions in the EHR and attend trainings.

Education

Master’s degree in a behavioral science or related field

Bachelor’s degree in a behavioral science or related field

30 college credits in behavioral sciences

High school diploma + 4,000 hours of experience
