Job summary

LifeBridge Health seeks a Clinical Performance Coordinator to support primary care practices in advancing high-quality, value-based care delivery. You will engage patients, coordinate care, and drive performance improvement across AWVs and preventive services.

You will work on documentation, data tracking, and reporting, partnering with providers and teams to close care gaps and improve outcomes in the Baltimore area.

Qualifications

• Two (2) to four (4) years of experience in healthcare operations, population health, care coordination, medical office workflows, quality improvement, or a related healthcare setting.

Responsibilities

• Identify and outreach to attributed and unaligned patients to strengthen relationships with primary care providers.
• Promote patient engagement through proactive communication and care coordination activities.
• Review daily ADT reports and coordinate timely follow-up after hospitalizations or ED visits.
• Assist with documentation to support quality measures and value-based reimbursement.
• Monitor KPIs, dashboards, and population health metrics to track progress.
