Summit Health, in partnership with VillageMD, is seeking an Associate, Population Health to help transform primary care delivery across our network. You will support value-based care initiatives, collaborate with clinical and operations teams, and help implement population health management strategies.

The role emphasizes data-driven improvements, wellness outreach, and building strong relationships with practice staff, with opportunities to grow across our care organizations.

Formación

• Bachelor’s degree in healthcare or a relevant field required.
• Experience in Population Management and Value-Based Care operations, including Medicare, Medicaid and Commercial lines of business.
• Excellent written & verbal communication skills and strong relationship-building abilities.

Responsabilidades

• Collaborate with VBC operations team to develop strategic operational solutions for improving quality, while reducing the cost of care delivered.
• Assist Manager of Value Based Care Operations in coordinating gap closure campaigns, and other market driven initiatives.
• Support outreach efforts for annual wellness visits.
• Build working relationships with Front Office Staff, Clinical Leads and Medical Assistants.
• Support successful practice performance under value-based contracts.
• Create performance reporting based on available data and system dashboards to help drive strategy.
• Other duties as assigned by Manager.
