Family Home Health Network is seeking a dedicated Home Health Nurse with a strong commitment to quality patient care in the Chicagoland area. You will perform initial assessments of patients, families, and home environments, monitor nursing needs, and initiate and revise the plan of care as patient status changes.

Requirements include current Illinois RN licensure, CPR, and fluent Polish; Home Health experience is preferred. Must have a valid IL driver’s license and auto insurance.

Qualificações

• Must possess current Illinois RN licensure.
• Home Health experience preferred.
• Must speak fluent Polish.

Responsabilidades

• Perform a detailed initial assessment of the patient, the family, and the home environment to determine home health care needs.
• Monitor and regularly re-evaluate the patient\'s nursing needs.
• Initiate the plan of care and make revisions as patient status changes.
• Coordinate care with other health professionals as needed.
