Job summary

Family Home Health Network in the Chicagoland area is seeking a dedicated Home Health Nurse with a strong commitment to quality patient care. You will perform a detailed initial assessment of the patient, family, and home environment to determine specific home health care needs and initiate the plan of care accordingly.

The ideal candidate is an Illinois RN with CPR certification, excellent clinical nursing and customer service skills, and fluent Polish.

Qualifications

• Must possess current Illinois RN licensure.
• Home Health experience preferred.
• Current CPR.
• Valid IL driver’s license and auto insurance policy.
• HCHB EMR experience a plus.
• Must possess excellent clinical nursing and customer service skills.

Responsibilities

• Complete a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs.
• Monitor and regularly re-evaluate the patient’s nursing needs and adjust the plan of care as patient status changes.
• Initiate the appropriate plan of care and make revisions as patient needs evolve.

is a provider of intermittent Home Health care, palliative care, hospice care and skilled/custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Home Health Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

• Salary determined on experience and licensure
