Job summary

City of Concord in Concord, NH, is seeking an Outreach Social Worker to engage with individuals and families, providing case management and referrals to shelter, mental health, and treatment services. The role works with police officers, school staff and community partners to deliver timely access to resources.

The position supports the ACERT program, leads outreach deployments, maintains partner relationships, and ensures compliant documentation while supporting a trauma-informed approach.

Qualifications

• Bachelor's degree in Social Work or related field with 3 years of related experience.
• Prior home visiting experience preferred.
• Experience collaborating with public safety organizations preferred.

Responsibilities

• Provide case management and referral services to individuals and families.
• Coordinate ACERT deployments and maintain partnerships with referral partners.
• Maintain databases and ensure confidentiality per policy.
• Collaborate with police, school staff and community organizations.
• Deliver outreach and connect clients to housing, mental health and substance use resources.
