Job summary

Legacy Care is seeking a compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to deliver talk therapy and medication management to residents in post-acute, long-term care, and assisted living settings in Richmond, VA.

This part-time role offers flexible scheduling and on-site visits with a focus on patient-centered, trauma-informed care, collaborating with nursing and primary care teams to support mental health across facilities.

Qualifications

• Master’s or doctoral degree from an accredited nurse practitioner program with a psychiatric mental health specialty.
• Current PMHNP board certification.
• Active, unrestricted Virginia licensure appropriate to the role and current DEA registration as required for prescribing.
• Current BLS certification and any additional credentials required by assigned facilities.
• Ability and willingness to provide talk therapy alongside medication management, with experience in supportive psychotherapy or other evidence-based approaches preferred.

Responsibilities

• Conduct comprehensive psychiatric assessments, mental health screenings, and risk evaluations.
• Provide individual talk therapy when needed, including supportive psychotherapy and other evidence-based interventions.
• Prescribe and manage psychiatric medications within your scope of practice, monitoring response and interactions.
• Coordinate referrals for additional therapy or specialty care as needed.
• Collaborate with nursing teams, primary care providers, social workers, and other clinicians on care planning.
