Legacy Care in Richmond, VA seeks a compassionate PMHNP to provide talk therapy and medication management in post-acute, long-term care, and assisted living settings.

This part-time role offers flexible scheduling and on-site visits, collaborating with nursing staff and interdisciplinary teams to address depression, anxiety, grief, and adjustment to facility living. Your autonomous practice supports residents’ emotional health with patient-centered care.

Qualifications

• Master’s or doctoral degree from an accredited PMHNP program.
• Current PMHNP board certification.
• Active VA licensure and current DEA registration for prescribing.
• Current BLS certification.
• Experience in geriatrics or long-term care preferred.
• Strong clinical judgement and documentation skills.

Responsibilities

• Conduct comprehensive psychiatric assessments and mental health screenings.
• Provide talk therapy and prescribe/manage psychiatric medications as appropriate.
• Collaborate with nursing staff and interdisciplinary teams in care planning.
• Document assessments and treatment plans accurately in the EMR.
• Educate patients, families and staff about mental health options and care strategies.
