Qualifications

Responsibilities

• Conduct behavioral health assessments for residents in long-term care.
• Develop and oversee individualized treatment plans with residents, families, and teams.
• Prescribe and manage medications and therapies in the long-term care setting.
• Provide counseling and support to residents and families.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents and staff about behavioral health and available resources.
