Integrity Care Group is seeking a Psychiatric Mental Health Practitioner or Physician Assistant to provide behavioral health services for residents in residential, skilled, group, and other long-term care facilities. The role includes initial evaluations, follow-up visits, and collaboration with nursing leadership to coordinate patient care.

The position requires board certification in psychiatric care, strong communication skills, and the ability to work effectively in a multidisciplinary team.

Qualifications

• Must be credentialed as a Nurse Practitioner or Physician Assistant.
• Board-certified in Psychiatric Mental Health practice or equivalent.
• Strong communication and teamwork abilities.

Responsabilités

• Perform initial psychiatric evaluations for residents in long-term care facilities.
• Provide routine follow-up visits and monitor medication efficacy and safety.
• Document visits per state and federal guidelines.
• Coordinate care with DON and other providers as needed.
• Respond to requests for psychiatric consultation and provide clinical direction.
