Job summary

Trio Health Partners seeks a compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to deliver psychiatric services in post-acute and long-term care settings. You will collaborate with a multidisciplinary team across multiple sites, travel required, and provide person-centered care to adults and geriatrics.

Requires active Illinois NP license and PMHNP-BC certification, a Master's in Nursing, and experience with geriatric mental health.

Qualifications

Active, unencumbered Illinois NP license.
Board-certified PMHNP (PMHNP-BC).
Master’s degree in Nursing from an accredited program.
Experience treating mental health conditions in adult/geriatric populations preferred.
Comfortable working in skilled nursing and long-term care environments.
Strong interpersonal, organizational, and communication skills.

Responsibilities

Conduct follow-up psychiatric visits after initial assessments.
Prescribe medications and review diagnostic results as needed.
Participate in team meetings and family sessions.
Collaborate with supervising psychiatrists and facility staff to implement treatment plans.
Maintain timely documentation and communication with care teams.
Provide education to facility staff regarding psychiatric care and patient needs.
Evaluate effectiveness of prescribed medications and make adjustments.
Ensure compliance with CMS guidelines and facility policies.
