Job summary
Trio Health Partners is seeking a Psychiatric Mental Health Nurse Practitioner to join our multi-site team with travel between facilities. The role focuses on delivering person-centered psychiatric care in post-acute and long-term care settings alongside a multidisciplinary team.
The ideal candidate holds an Illinois PMHNP license (PMHNP-BC) with a master's in nursing, and can collaborate with physicians, facility staff, and families.
Qualifications
- Active, unencumbered Illinois NP license.
- Board-certified PMHNP (PMHNP-BC).
- Master's degree in Nursing from an accredited program.
- Experience treating adult/geriatric mental health conditions.
- Comfortable in skilled nursing and long-term care environments.
- Strong interpersonal, organizational and communication skills.
Responsibilities
- Conduct follow-up psychiatric visits after initial assessments.
- Prescribe medications and review diagnostic results as needed.
- Participate in team meetings and family sessions.
- Collaborate with supervising psychiatrists and facility staff to implement treatment plans.
- Maintain timely documentation and communication with care teams.
- Provide education to facility staff regarding psychiatric care and patient needs.
- Evaluate effectiveness of prescribed medications and make adjustments.
- Ensure CMS guidelines and facility policies are followed.
