Qualifications

• Active New York State Nurse Practitioner License is required.
• DEA Registration is required.
• Psychiatric Mental Health Nurse Practitioner (PMHNP) certification is required.
• Experience working with homeless, behavioral health, substance use, or underserved populations is preferred.
• Experience in integrated primary care or community health settings is preferred.
• Commitment to serving underserved populations and ability to work in non-traditional settings is essential.
• Excellent communication and collaboration skills are essential.

Responsibilities

• Provide psychiatric evaluation, diagnosis, treatment, and medication management to patients.
• Engage and support clients with sensitivity to behavioral health needs and treatment goals.
• Coordinate care with Primary Care Providers for chronic and acute conditions.
• Counsel patients regarding mental and physical health needs and treatment plans.
• Collaborate with case management, health education, and interdisciplinary teams.
• Assist in identifying and coordinating care for individuals with mental health and substance use disorders.
• Maintain timely and accurate documentation per CFH policies and procedures.
• Participate in peer review activities and committee assignments as requested by the CMO.
• Provide consultation, education, and training to staff and volunteers as appropriate.
• Deliver services in non-traditional settings including shelters and community-based locations.
