Responsibilities

• Provide psychiatric evaluation, diagnosis, treatment, and medication management to patients.
• Engage and support clients with sensitivity to behavioral health needs and treatment goals.
• Coordinate care with Primary Care Providers for chronic and acute conditions.
• Counsel patients regarding mental and physical health needs and treatment plans.
• Collaborate with case management, health education, and interdisciplinary teams.
• Assist in identifying and coordinating care for individuals with mental health and substance use disorders.
• Maintain timely and accurate documentation per CFH policies and procedures.
• Participate in peer review activities and committee assignments as requested by the CMO.
• Provide consultation, education, and training to staff and volunteers as appropriate.
• Deliver services in non-traditional settings including shelters and community-based locations.

Position Summary

The Psychiatric Nurse Practitioner (PMHNP) provides comprehensive behavioral health services as part of the Care for the Homeless' Patient-Centered Medical Home (PCMH) team. Reporting to the collaborating physician, the PMHNP delivers psychiatric assessment, treatment, care coordination, and patient education to underserved populations.
