Job summary

Impact Health PC in Lancaster, PA is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric care in Skilled Nursing Facilities. You will assess, diagnose, and manage psychiatric conditions and develop individualized treatment plans.

You will collaborate with physicians and interdisciplinary teams, document thoroughly in EHRs, and enjoy a flexible schedule with strong clinical support and competitive compensation.

Qualifications

• Active PMHNP license/certification in NJ as applicable.
• Minimum 2 years of clinical experience providing mental health care to adults.
• Experience in Skilled Nursing, Long-Term Care, Geriatrics, Hospital, Residential Care, or Outpatient Psychiatry preferred.

Responsibilities

• Provide comprehensive psychiatric evaluations and ongoing medication management.
• Assess, diagnose, and treat psychiatric and behavioral health conditions.
• Develop evidence-based, individualized treatment plans.
• Collaborate with physicians, nursing staff, and interdisciplinary care teams.
• Provide patient and family education regarding diagnoses, medications, and treatment plans.
• Complete timely clinical documentation and maintain compliance with applicable regulations.
