Job summary

Qualifications

• Unrestricted PMHNP license.
• Knowledge of standard documentation practices.
• Experience with geriatric population and dementia preferred.
• Experience in post-acute or healthcare setting preferred.

Responsibilities

• Optimize quality of life for residents in skilled nursing, assisted living and/or long‑term care settings working alongside an interdisciplinary team that includes medical professionals, psychotherapists and facility staff.
• Assessment, diagnosis, treatment and medical management of conditions including anxiety, depression, adjustment disorder, and pre‑existing mental illnesses.
• Maintain timely, accurate documentation.
• Provide education to facility staff regarding psychiatric care and patient needs.
• Ensure compliance with CMS guidelines and facility policies.
