Job summary

UltaCare Medical is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) for post-acute care settings, offering autonomy and flexible schedules. The PMHNP will provide assessment, diagnosis, and management of mental health conditions for residents in skilled nursing, assisted living, and long-term care facilities.

Ideal candidates hold an unrestricted PMHNP license and have experience with geriatric populations and post-acute care.

Qualifications

• Unrestricted PMHNP license.
• Knowledge of standard documentation practices.
• Experience working with geriatric populations and dementia.
• Experience in post-acute or healthcare settings.

Responsibilities

• Optimize quality of life for residents in skilled nursing, assisted living and long-term care settings with an interdisciplinary team.
• Assessment, diagnosis, treatment and medical management of mental health conditions.
• Maintain timely, accurate documentation.
• Provide education to facility staff regarding psychiatric care and patient needs.
• Ensure CMS guidelines and facility policies are followed.
