Job summary

UltaCare Medical is seeking a Psychiatric Mental Health Nurse Practitioner to provide expert psychiatric care in post-acute settings such as skilled nursing and long-term care facilities.

You will work with an interdisciplinary team to improve patient outcomes, conduct assessments, and manage treatment plans while maintaining thorough documentation and adherence to CMS policies.

Qualifications

• Knowledge of geriatric care needs and dementia considerations.
• Familiarity with CMS guidelines and documentation standards.
• Experience in post-acute or healthcare settings considered a plus.

Responsibilities

Essential Functions and Responsibilities:

• Optimize quality of life for residents in skilled nursing, assisted living and/or long-term care settings working alongside an interdisciplinary team that includes medical professionals, psychotherapists and facility staff.
• Assessment, diagnosis, treatment and medical management of conditions including anxiety, depression, adjustment disorder, and pre-existing mental illnesses.
• Maintain timely, accurate documentation.
• Provide education to facility staff regarding psychiatric care and patient needs.
• Ensure compliance with CMS guidelines and facility policies.
