Job summary

CarePlus New Jersey has a leading role in mental health care in Northern New Jersey, offering integrated services for adults with behavioral health needs. This PMHNP role is part of a multidisciplinary CRSC team delivering on-site holistic nursing care 24/7, including psychiatric evaluations and medication management.

You will prescribe medications per FDA guidelines, provide education to clients and families, and collaborate with the team to ensure continuity of care.

Qualifications

• MSN degree or higher.
• Current active, unencumbered New Jersey Advanced Practice Nurse license.
• Current National Certification as Nurse Practitioner. Psychiatric Mental Health Nurse Practitioner (PMHNP) certification.
• Must be a registered practitioner with the US DEA.
• Active CDS.
• Minimum two years experience treating SMI, SUD, and/or COD.
• Experience providing medication management services in a community behavioral health center.

Responsibilities

• Conduct comprehensive evaluations of individuals and ensure that the full range of behavioral considerations are accounted for when determining diagnosis and treatment.
• Order appropriate laboratory and diagnostic tests as indicated by individual's presentation and medications prescribed.
• Prescribe medications as indicated by signs, symptoms, and diagnosis, and provide medication monitoring.
• Utilize evidence-based practices in psychotherapeutic and clinical interventions.
• Document interventions in a timely and accurate manner
• Review treatment plans, monitor changes in health status, provide health coaching and support, administer long-acting injectable antipsychotics, and respond clinically to abnormal physical findings.
• Work with CRSC team to support population health management.
• Participate in continuous quality improvement initiatives.
• Offer education to clients and their families regarding medications, risks, benefits, and expected outcomes.
• Practice both in-person and via telemedicine.
• Participate in weekly supervision and team meetings.
