Job summary

Cordial Health is seeking a compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) or a Psychiatric experience- skilled Physician Assistant to provide on-site behavioral health care across Northern New Jersey. The role offers a three-day-per-week schedule and meaningful patient relationships with strong clinical and operational support.

Qualifications

• Board-certified PMHNP-BC or PA with significant psychiatric experience.
• Active and unrestricted New Jersey professional license.
• Active DEA registration required.
• Strong psychiatric assessment, diagnosis and psychopharmacology skills.
• Ability to manage a psychiatric caseload independently.
• Familiarity with electronic medical record systems.
• Geriatric, SNF, rehab or post-acute experience preferred.

Responsibilities

• Conduct psychiatric evaluations, consultations and follow-up visits.
• Diagnose and manage psychiatric and behavioral health conditions.
• Develop individualized, evidence-based treatment plans.
• Prescribe, monitor and adjust psychotropic medications.
• Evaluate treatment response, medication effectiveness and side effects.
• Collaborate with physicians, nursing staff, therapists, social workers and families.
• Participate in interdisciplinary treatment planning.
• Educate residents, caregivers and staff regarding psychiatric care.
• Provide compassionate, trauma-informed and resident-centered treatment.
• Complete accurate and timely clinical documentation.
