Job summary

Skilled Nursing Pharmacy in Hayward, CA is seeking a dedicated Staff Pharmacist to join our team, delivering high-quality pharmaceutical care to skilled nursing facility residents. You will work to ensure optimal medication therapy and patient safety in a collaborative long-term care setting.

In this role, you will dispense and verify medications, perform DUR, and partner with nurses and physicians to optimize regimens.

Qualifications

• Active California Pharmacist license (RPh) in good standing.
• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Experience in long-term care, skilled nursing, or institutional pharmacy preferred.
• Strong knowledge of geriatric pharmacotherapy and medication management.
• Familiarity with California Board of Pharmacy regulations and OBRA guidelines.

Responsibilities

• Dispense and verify prescription medications accurately for skilled nursing facility residents.
• Perform drug utilization reviews (DUR) and ensure medication therapy appropriateness.
• Collaborate with nursing staff and physicians to optimize patient medication regimens.
• Monitor for drug interactions, contraindications, and adverse drug reactions.
• Ensure compliance with state and federal pharmacy regulations and long-term care guidelines.
• Counsel and support facility staff on medication administration and safety protocols.
• Maintain accurate patient medication records and documentation.
