Qualifications

• Pharmacist with PharmD degree and active CA license.
• Minimum of 2 years of experience in a clinical pharmacy setting.
• Strong communication and interpersonal skills; ability to work collaboratively in fast-paced environments.

Responsabilités

• Review and interpret medication orders for accuracy and appropriateness.
• Provide drug information and counseling to patients and healthcare professionals.
• Monitor patient medication therapy for safety and efficacy.
• Participate in interdisciplinary team meetings to optimize patient care.
• Ensure compliance with state and federal pharmacy regulations.
• Prepare and dispense medications accurately and efficiently.
• Conduct medication reconciliation to prevent errors.
• Educate staff on medication management best practices.

About the Role:

Join Skilled Nursing Pharmacy as a Pharmacist, where you will play a vital role in providing exceptional pharmaceutical care to our patients. With a focus on improving health outcomes, you\'ll collaborate with a dedicated team in a supportive environment that values your expertise.

Available Schedule:

• PM Shifts starting times vary between 4:00pm - 8:00pm.


• PM Shifts will include a Shift Differential.

Responsibilities:
