Job summary

Valley Vista Post-acute is seeking a Licensed Vocational Nurse (LVN) for the PM shift in Escondido, CA. The LVN assists RNs, organizes day-to-day nursing duties, and ensures high quality care in accordance with regulations.

You will monitor residents' health, administer medications, participate in care planning, and collaborate with CNAs and other health professionals to support resident well-being.

Qualifications

• Must be a licensed LVN or LPN in California (CA).

Responsibilities

• Conducts initial and ongoing assessments of residents' health status.
• Monitors vital signs and administers medications as prescribed.
• Notifies the RN or healthcare provider of significant changes or concerns.
• Administers medications as prescribed.
• Ensures accurate dosage, proper route, and timely administration.
• Monitors and records residents' responses to medications.
• Provides basic wound care and other treatments as ordered by the provider.
• Monitors healing progress and reports complications.
• Collaborates with RNs, CNAs and other staff to coordinate care.
• Participates in care planning meetings and helps develop resident care plans.
