Job summary

Comprehensive Rehab Consultants (CRC) seeks a part-time PM&R clinician (NP/PA) to provide physiatry services at a designated SNF/post-acute rehab center in Charlotte, NC. You will lead clinical care, collaborate with PCPs and therapy teams, and drive patient independence.

You will join a physician-owned group offering training, growth, and opportunities to advance within CRC, with flexible weekday hours and minimal on-call requirements.

Qualifications

• Experience in post-acute space preferred.
• Ability to work autonomously in SNF/post-acute settings.
• Strong interpersonal and collaboration skills with inter-disciplinary team.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute facility.
• Represent CRC in facility through clinical leadership and collaboration with care team.
• Collaborate with PCP teams to manage common SNF conditions such as CVA and postoperative surgeries.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
