Comprehensive Rehab Consultants seeks a PM&R Clinician (NP/PA) to deliver rehabilitation services at a designated SNF/post-acute center in North Cape May, NJ. This full-time role supports a collaborative care model with PT/OT/SLP teams.

Responsibilities include leading clinical care, partnering with PCPs, and promoting patient independence while benefiting from education opportunities and comprehensive benefits.

Qualifications

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility with clinical leadership and interdisciplinary team.
• Collaborate with PCP teams to manage SNF conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Follow patients to track progress in therapy.
