You will administer medications, perform assessments, collaborate with CNAs and families, and help maintain accurate documentation while supporting safe, dignified care for residents.

Qualifications

• Current Wisconsin RN or LPN license required.

Responsibilities

• Provide professional nursing care to residents in our skilled nursing and rehabilitation community.
• Administer medications and treatments according to physician orders and facility standards.
• Complete assessments, monitor resident conditions, and communicate changes to providers and the interdisciplinary team.
• Collaborate with CNAs, families, providers, and team members to support individualized resident care.
• Support resident care plans and promote resident safety, comfort, and dignity.
• Complete accurate and timely clinical documentation.
• Provide direction, support, and leadership to nursing assistants to promote quality resident outcomes.
