Mozaic (A Chapter of Arc NY) is seeking a compassionate and organized Planning Coordinator to support people receiving services within our Day Services, Day Habilitation, and Community Habilitation programs throughout Yates and Seneca Counties. In this role, you will help ensure people we support receive high-quality, person-centered services that promote independence, community inclusion, and achievement of their personal goals.

Planning Coordinator Duties

The Planning Coordinator is responsible for ensuring regulatory and agency compliance while supporting people through person-centered planning and service coordination. Responsibilities include monitoring and maintaining Life Plans, ensuring accurate and timely documentation, overseeing program budgets and individual funds in accordance with agency procedures, and advocating for the people supported. This role promotes community integration and develops positive relationships with families, service providers, employers, and community partners while upholding Mozaic's mission, vision, and values. The Planning Coordinator also safeguards the privacy, dignity, and confidentiality of people receiving services.

Planning Coordinator Skills

This position requires strong organizational skills, attention to detail, and the ability to manage multiple priorities independently. The successful candidate will demonstrate professionalism, strong communication and teamwork skills, sound judgment, and discretion when handling confidential information. Proficiency with computers and office technology is required. A valid New York State Driver's License is required, including the ability to operate agency vehicles and wheelchair-accessible vans. A working telephone must be maintained, with updated contact information provided to the agency.

Salary Range

$19.53 - $24.63 per hour

In addition to competitive pay, employees are eligible for mileage reimbursement when traveling to Seneca County from the home base location in Penn Yan in accordance with agency policies and procedures.

235 North Avenue, Penn Yan, NY

Serving participants throughout Yates and Seneca Counties

Schedule

Monday-Friday, 8:00 a.m. - 4:00 p.m.

Flexibility required to meet the needs of the individuals and programs served.

37.5 hours per week

Planning Coordinator Qualifications:

High School diploma or equivalent required and 1 year of experience working with people with developmental disabilities.
