The Placement Coordinator serves as a key member of the Clinical Case Management team and is responsible for facilitating timely and appropriate post-acute care transitions through comprehensive referral management, placement coordination, authorization support, and transportation arrangements. Working collaboratively with Clinical Case Managers, physicians, nursing, post-acute providers, and community partners, the Placement Coordinator manages post-acute referrals, ensuring patients are transitioned to the appropriate level of care while minimizing discharge delays and supporting optimal patient outcomes.
The Placement Coordinator functions as a liaison between UCDH, post-acute providers, placement agencies, payers, and interdisciplinary teams to ensure efficient communication, coordination, and follow-through of post-discharge services. This role is responsible for managing referral workflows through a referral management system, obtaining post-acute authorizations, coordinating patient transportation through a system integrated in the electronic medical record, supporting difficult-to-place patients, and identifying resources that facilitate safe and timely discharges. The Placement Coordinator contributes to departmental initiatives focused on patient throughput, length‑of‑stay reduction, and care transitions.

The Clinical Case Management Department has the combined responsibilities of hospital-wide Discharge Planning, Utilization Review and MediCal TAR Free Processing. The Clinical Case Management purpose is to reduce hospital costs, monitor length of stay and appropriateness of medical care in the acute setting, ensure reimbursement through timely medical review and/or obtaining authorizations for services rendered, provide discharge planning interventions to assure timely and safe patient discharge, facilitate outpatient referrals or acute transfer and urgent direct admission requests, refer patients to and assist in the application of special programs when needed.
