Qualifications

Job description

Role Overview: This full-time Physician position involves providing comprehensive primary care across multiple community-based settings within a multi-center program dedicated to older adults' health and independence.

• Responsibilities: Deliver medical, behavioral health, therapeutic, and recreational services in outpatient clinics, private residences, assisted living, adult care homes, and skilled nursing facilities. Collaborate with an interdisciplinary team including physicians, nurse practitioners, social workers, pharmacists, and mental health clinicians. Maintain continuity of care by temporarily managing patient panels. Foster trust and confidence with patients and families.
• Qualifications & Skills: Resourceful, clinically confident, experienced in geriatric care preferred, with training in palliative or hospice care advantageous. Strong interpersonal skills and ability to work in diverse community settings.
• Key Duties: Provide patient-centered care, coordinate with team members, and support older adults' well-being and independence.
