Job summary

HarmonyCares provides in-home primary care for patients with complex health needs. This physician role focuses on delivering high-quality, patient-centered care in a home setting, requiring an active medical license and DEA, with EMR experience and geriatrics expertise.

The position supports patients across multiple states, coordinating with a team-based, physician-driven model to improve access to care and outcomes for high-acuity patients at home.

Qualifications

• Experience in Geriatrics or complex patients preferred.
• Active/unrestricted medical license and DEA.
• Board Certified or Board Eligible.
• EMR experience.
• Must maintain a valid driver’s license and good driving record.
